Activities That Engage My Child

The purpose of this online form is to have a more complete "picture" of your child that will assist my instructional assistant, Catrina, and myself in modifying our instructional strategy to better accommodate our student's needs.  To accomplish this, we need to know more about those activities that engage your child, both at home and other settings (i.e., restaurant, grocery store).  Completing this form should not take you any longer than 15 minutes and please collaborate with other care providers who come in contact with your child.

What do I mean by engage?  These are activities that your son or daughter engage in that require little or no redirection (verbal or visual prompting).  Novel activities are nice to note, however, those activities that are more common place need to be addressed.  When you complete this online form, your information will be sent to my e-mail address after you click on the submit button and will be included in his/her confidential records.  If you have any questions, please contact me at home (760) 598-1701 or at school (760) 630-4870.  Now, lets begin:
